If you have forgotten your login credentials and are unable to log in to the OQ®-Analyst site or have locked your account, you may contact OQ Measures Support at the contact information below. Also any System Admin user at your Organization can also reset your password or unlock your account so long as they have permissions in your assigned Clinic.
